Hospice helps patients and their families prepare physically, emotionally and spiritually for the end of life. Our focus is on providing comfort, not cure; on family, not just the patient; and on quality of life, not duration. Cleveland Clinic’s hospice team includes physicians certified in hospice and palliative medicine, nurses, social workers, chaplains, home health aides and volunteers. 
 

A caregiver in this PRN position may work varying shifts – days, afternoons or midnights.

This position will work on the west site of Cleveland/ Lakewood-area.
 

A caregiver who excels in this role will:  

  • Perform basic patient care activities.  

  • Assist with procedures ordered by physician and supervised by an RN, PT, OT or ST as appropriate.  

  • Assist patients with tending to personal care, activities of daily living and transfers/transport. 

  • Report and document abnormal assessment findings or changes in physical, mental and emotional conditions to nursing staff. 

  • Assist with keeping the patient's environment clean and orderly. 

  • Maintain complete and up-to-date documentation and files at all times regarding patient condition and effectiveness and timeliness of care and interventions. 

  • Escalate change in patient condition to patient’s physician as appropriate.  

Minimum qualifications for the ideal future caregiver include:  

  • High school diploma or GED.  

  • Certification and registered with the State of Ohio Nurse’s Aide Registry (STNA).  

  • One year of clinical experience as an aide in an acute, rehab or skilled nursing setting. 

  • Valid Ohio driver’s license and proof of automobile insurance coverage with $100,000/$300,000 coverage.  

Preferred qualifications for the ideal future caregiver include: 

  • Basic Life Support (BLS) certification through the American Heart Association (AHA). This will be required within the orientation period prior to providing independent patient care.  

  • Hospice and Palliative Care Nurse Assistant (CHPNA) certification.  

  • Home care experience.

Physical Requirements:

  • Must be ambulatory, able to climb stairs, pull, push, and facilitate movement of the patient.
  • For field staff only: Able to drive to patients’ homes and/or facilities.
  • Medium Work – Exerting 20 to 50 pounds of force occasionally, and/or 10 to 25 pounds of force frequently, and/or greater than negligible up to 10 pounds of force constantly to move objects.
  • Physical Demand requirements are in excess of those for Light Work.

  • Follows standard precautions using personal protective equipment as required.
