This position works closely with other transplant social workers and a social worker lead. You will report to the Clinical Manager for Lung Transplant. The ideal caregiver will have experience with patients with chronic health issues and patients requiring long term follow up.
A caregiver in this position works onsite, 8:00am—5:00pm, with 24-hour on call requirements.
A caregiver who excels in this role will:

Complete psychosocial assessment for selection of a candidate for organ transplant and living donor.

Conduct psychosocial assessment and interventions and makes treatment recommendations and engages patients/families in adherence to the treatment plan by reducing or eliminating, to the extent possible, the social, psychological, financial, behavioral, and regulatory barriers to successful medical outcomes.

Deal with families exhibiting complex family dynamics.
-
Communicate with interdisciplinary team regarding the discharge planning status of all patients referred by them.
-
Educate patients/family and physician regarding post-acute options and address freedom of choice and financial concerns.
-
Participate in discharge planning for assigned patients to ensure a timely discharge and provide appropriate linkage with post-discharge care providers and resources.

Serve as a resource person and provides counseling and intervention related to treatment decisions and end-of-life issues.

Provide intervention in cases involving child abuse/neglect, domestic violence, elderly abuse, institutional abuse, and sexual assault.

Advocate for patient and family empowerment and independence to make autonomous health care decisions and access needed services within the health care system.

Stay current with regulations which impact the care of patients and the Clinic's regulatory compliance.

Promote individual professional growth and development and meets mandatory/continuing education, skills competency, supports department-based goals which contribute to the success of the organization; serves as a preceptor, mentor, and resource to less experienced staff.
Minimum qualifications for the ideal future caregiver include:

Master's degree in social work OR Master's of Science in Social Administration (MSSA).

Obtain a state licensure as a Social Worker LISW or LISW-S within 27 months of hire date.

Exposure and experience in pre-acute and post-acute ca re.
-
Excellent communication and negotiation skills necessary to work collaboratively with multidisciplinary teams for optimal outcomes.

Can work independently and in teams.
Preferred qualifications for the ideal future caregiver include:

Hospital Social Work experience a plus
