The Community Health Driver promotes a positive Cleveland Clinic Experience through friendly, courteous, safe and effective transport of the school-based health mobile unit to the various schools within Cuyahoga County and participates as a key team member of our School Based Health team. In this role, you will help build and fosters relationships with the clinical team, administrative team, and community stakeholders to deliver medical services through the mobile unit. This role offers continuous professional development through CCF resources and helps address care gaps in our local communities.
A caregiver in this position works Monday-Friday 6:00am-2:30pm during the school year and flexible shifts during the summer.
A caregiver who excels in this role will:

Provide safe, efficient, and quality mobile unit transportation service as measured by driving record, compliance with established DOT regulations and Cleveland Clinic safety policies.

Complete daily vehicle safety inspections prior to placing the mobile unit into operation and at the end of the shift.

Maintain currency and effectiveness in operating wheelchair lifts and associated equipment. Safely secures patient and wheelchair at the wheelchair station.

Maintain situational awareness both inside/outside the Mobile Unit at all times.

Assist students and patients on and off the mobile unit.

Conduct friendly, courteous and helpful interactions with students, families, school district and community members.

Maintain vehicle presentation and cleanliness at all times in accordance with policy and procedures.

Perform basic patient registration and scheduling.

Enter and maintain confidential and accurate patient information including patient insurance, billing, and payment information.

Recruit, enroll, and educate patients on MyChart.

Support and participate in process improvement efforts.

Effectively collaborates and communicates with clinical staff, i.e. providers, nurses and other support staff.

May participate in patient outreach activities such as proactive patient calls to assist with health maintenance and care compliance.

Other duties as assigned.
Minimum qualifications for the ideal future caregiver include:

High School Diploma or GED required.
-
Must possess a valid State Commercial Drivers License (CDL) with air brakes and passenger endorsement and have less than 6 points on record.

Driver must maintain eligibility of insurability as determined by CCF's insurance carrier's requirement to operate Foundation vehicles.

Drivers assume responsibility to pay fees associated with traffic violations.
-
Working knowledge of PC window applications sufficient to input and retrieve data.

Ability to partner with others as needed to troubleshoot and resolve general technology issues.
Preferred qualifications for the ideal future caregiver include:

Minimum five years driving experience; prior experience transporting passengers, patients or clients preferred.

Demonstrated experience in providing exceptional customer services and computer skills.

Physical Requirements:

Typical physical demands include ability to assist physically challenged patients in wheelchair on and off wheelchair left on vehicle.

This includes using both wheelchair lift and ramp equipped vehicles.

Driver must be capable of exerting push force of 45lbs.

Must be able to pass an annual visual and hearing test.
