As an LPN in one of our Primary Care Clinics, you will use your nursing knowledge and skills to assist with providing timely, effective and efficient patient-centered care. A caregiver in this position works days from 8:00AM – 5:00PM.
A caregiver who excels in this role will:
- Under the direction of a Registered Nurse, physician, optometrist and/or podiatrist, perform patient care activities and complete required documentation for patients of all ages.
- Collect and document objective and subjective data and observations about the patient.
- Contribute observations and health information to the nursing assessment and report all data to the RN or authorized directing health care provider.
- Implement the current plan of nursing care at the direction of the RN, or the medication or treatment authorized by the authorized directing health care provider.
- Document and verbally communicate the patient’s response to the nursing plan of care, or the medication or treatment including observing patients for adverse reactions to medications or treatments and performing routine laboratory tests.
- Contribute to the revision of the nursing plan of care
- Provide education and information to patients and family members.
- Maintain professional growth and development within nursing field.
- Participate in the departmental orientation of new caregivers in a positive manner, share knowledge and experiences, and provide guidance when assigned.
Minimum qualifications for the ideal future caregiver include:
- Graduate of an approved School of Practical Nursing
- Current Ohio licensure as a Licensed Practical Nurse (LPN)
- Basic Life Support (BLS) Certification through the American Heart Association (AHA)
Preferred qualifications for the ideal future caregiver include:
- One year of patient care experience

Physical Requirements:
- Full body motion to move and lift patients
- Manual finger dexterity with good hand-eye coordination
- Extensive standing walking and occasional lifting
- Corrected vision and hearing to normal range
- Working under stressful conditions or working irregular hours
- Some exposure to communicable diseases or body fluids
- Medium Work, exerting 20 to 50 pounds of force occasionally and / or 10 to 25 pounds of force frequently and / or greater than negligible up to 10 pounds of force constantly to move objects
- Physical Demand requirements in excess of those for Light Work
