As a Healthy Start Homecare Nurse, you will participate in a team-based care model led by the Care Coordinator, providing top-quality maternal and child healthcare to the community. A caregiver in this position works days from 8:00AM – 4:30PM.
A caregiver who excels in this role will:
- Provide direct nursing care and perform accurate and complete starts of care, resumptions, recertifications and routine visits as requested and within specified time frames.
- Perform medication reconciliation at each visit.
- Assess patient for safety risks, abuse and neglect.
- Serve as a primary coordinator of all disciplines for well-coordinated patient care.
- Assess and document pain, manage pain and prioritize/document problems based on assessed needs and problems in collaboration with patient, family and health care team.
- Maintain complete and up-to-date documentation and file at all times regarding patient condition and effectiveness and timeliness of care and interventions.
- Escalate change in patient condition to physician as appropriate.
Minimum qualifications for the ideal future caregiver include:
- Graduate of an accredited school of Professional Nursing
- Current Basic Life Support (BLS) Certification through the American Heart Association (AHA)
- Current Neonatal Resuscitation Program (NRP) Certification through the American Academy of Pediatrics (AAP)
- Valid Driver's License (VDL) and proof of auto insurance coverage
- Two years of Prenatal Nursing experience in an acute care setting
- One year of clinical experience as a RN in an acute medical-surgical setting
Preferred qualifications for the ideal future caregiver include:
- Infusion Therapy experience
- Postpartum/OB experience

Physical Requirements:
- Requires full range of motion, manual and finger dexterity and eye-hand coordination.
- Requires standing and walking for extensive periods of time.
- Requires corrected hearing and vision to normal range.
- Requires some exposure to communicable disease or bodily fluids.
- Medium Work – Exerting 20 to 50 pounds of force occasionally, and/or 10 to 25 pounds of force frequently, and/or greater than negligible up to 10 pounds of force constantly to move objects.
- Physical Demand requirements are in excess of those for Light Work.
- Must be ambulatory and able to drive to patients' homes, able to climb stairs, pull, push, and facilitate movement of the patient.
- Must be able to read, write and communicate with patients.

- Follows standard precautions using personal protective equipment as required.
