This position is a part of the Specialty Pharmacy Team, which specializes in supporting patients with complex disease states and high-cost medications. This proactive team ensures patients receive personalized care by managing prior authorizations, patient assistance programs, and comprehensive medication education. Medications are only delivered after direct patient contact to confirm understanding and access. In this leadership role, you will drive key initiatives in patient care, medication safety, access, education, research, and regulatory compliance—while also guiding clinical workflows, supporting professional growth, and ensuring adherence to REMS programs and accreditation standards

Caregivers in this role will work Monday through Friday, for one of two 8-hour shifts between 7:00am to 6:00pm. There are various shifts and caregivers will rotate open and closing times. No weekend, no holidays and no on-call requirements .

A caregiver who excels in this role will:

  • Implements and standardizes clinical pharmacy services for patients with complex, chronic conditions requiring specialty medications.
  • Ensures timely access to high-cost therapies through proactive patient outreach, prior authorization, and patient assistance support.
  • Mentors pharmacists, residents, and students, and collaborates across teams to enhance clinical practice and patient outcomes.
  • Contributes to strategic planning by recommending new services, cost-saving initiatives, and process improvements.
  • Oversees quality assurance and supports compliance with REMS and regulatory standards.
  • Provides clinical direction under the manager’s guidance and participates in hiring decisions.
  • Maintains or is pursuing Board Certification in Pharmacotherapy or Ambulatory Care.
  • Performs other duties as assigned to support departmental goals. ​

Minimum Qualifications for the ideal future caregiver include:

  • Doctor of Pharmacy degree from an American Association of Colleges of Pharmacy (AACP) accredited university.
  • Current state licensure as a Registered Pharmacist (RPh) or eligible for reciprocation in current state within 90 days of hire.
  • For individuals required to administer immunizations,currentBasic Life Support (BLS)certification through American Heart Association (AHA) andcompletion of a Board of Pharmacy approved course inthe administration of immunizations or certificationobtained within 90 days of hire are required.  
    For individuals required to respond to adult or pediatriccardiopulmonary arrest events, currentAdvancedCardiovascular Life Support (ACLS) or PediatricAdvanced Life Support (PALS)Certification through American Heart Association (AHA), orcertification obtained within one (1) year of hire, is required.
  • Completion of a post-graduate year 2 residency (PGY2) plus a minimum of 1 year of experience providing pharmaceutical care in the relevant specialty area or
  • Completion of a post-graduate year 1 (PGY-1) residency plus a minimum of 3 years of experience providing pharmaceutical care in the relevant specialty area or
  • Minimum of 5 years of experience providing pharmaceutical care in the relevant specialty area

Preferred Qualifications for the ideal future caregiver include:

  • Board certification through the Board of Pharmacy Specialties, preferably in Pharmacotherapy or Ambulatory Care, strongly preferred at time of hire and required within one year
  • Previous leadership experience preferred
