The Social Worker BSW will help patients and families in achieving improved health outcomes by reducing the bio psychosocial-spiritual obstacles that interfere with their ability to use health resources effectively. The goal is to connect patients with needed services in the community and set them up for successful discharge to prevent readmission.
A caregiver in this position works days, 7:00am—3:30pm or 7:30am—4:00pm.
A caregiver who excels in this role will:

Assess patient and family's psychosocial risk factors through evaluation of prior functioning levels, appropriateness and adequacy of support systems, reaction to illness and ability to cope.

Assess patient and family's financial, social, emotional, educational, and environmental resources and identifies needs based on patient's current medical condition.

Develop an individualized plan of care based on the findings of the assessment and needs of the patient and family/caregiver as related to the patient's medical condition.

Participate in Discharge planning activities for complex patients to ensure a timely discharge and to provide appropriate linkage with post-discharge care providers.
Minimum qualifications for the ideal future caregiver include:

Licensed Social Worker (LSW) in the current State
-
Proof of Basic Life Support (BLS) through American Heart Association (AHA) must be presented upon hire
-
Excellent communication and negotiation skills necessary to work collaboratively with multidisciplinary teams for optimal outcomes

Show ability to work independently and with teams
-
Must be flexible, open-minded, and adaptable to change
Preferred qualifications for the ideal future caregiver include:

Three years of experience in healthcare systems to include exposure to and experience in pre-acute care
-
Social Worker OR Case Management Certification (CCM) (within one year of eligibility)

Hospital Social Work experience

Knowledge of community resources
