This Occupational Therapist provides in-home patient assessment and treatment, managing a caseload and following patients throughout their plan of care. The role supports individuals with a broad range of medical conditions, including orthopedic, cardiovascular, and neurological diagnoses, reflecting the diverse patient population served. Responsibilities include receiving the daily schedule in advance, coordinating directly with patients to arrange visit times and travel routes, and participating in team care conferences to support interdisciplinary care coordination and optimal patient outcomes. Caregivers in this specific role will provide support and care to our patients residing in Chagrin Falls, Chardon, Chesterland, Gates Mills, Middlefield, Newbury, Novelty and surrounding villages and townships.
A caregiver in this position will work 8:00am to 5:00pm. Saturday rotations occur every 5th Saturday. 1 holiday and 1 holiday weekend rotation will occur.
A caregiver who excels in this role will:
- Perform initial and ongoing assessments of patient’s condition.
- Establish, revise, and evaluate a plan of care which is appropriate to problems identified and involves the patient/family according to their capabilities and desires.
- Perform therapy interventions utilizing standard occupational therapy techniques and skills as appropriate for the age and condition of the patient.
- Supervise activities of occupational therapist assistants, students, and other support personnel.
- Always maintains complete and up-to-date documentation and files regarding patient condition and effectiveness and timeliness of care and interventions.
- Participate in ongoing interdisciplinary assessment of assigned patients. Participates as a member of the hospice interdisciplinary team when necessary.
Minimum qualifications for the ideal future caregiver include:
- Bachelor's degree
- Must be a graduate of an Occupational Therapy Program accredited by the American Occupational Therapy Association
- Current State Licensure as an Occupational Therapist (OT)
- Current Basic Life Support (BLS) through American Heart Association (AHA) or American Red Cross
- Valid Ohio Driver’s license upon hire; per policy 8-175 thereafter
- Proof of automobile insurance coverage with $100,000/$300,000 coverage, upon hire; per policy 8-175 thereafter
Preferred qualifications for the ideal future caregiver include:
- Experience in clinical area preferred
- Homecare experience
- Two years of experience in OT
Physical Requirements:
- Ability to walk without assistive device and to maintain balance despite sudden load to support patient from falling.
- Ability to meet physical demands characterized at the heavy level defined by 1993 Leonard Matheson and Ministry of Labor.
- Ability to lift and reach with arms sufficiently to lift patient's arm and leg during therapeutic treatment program.
- Sensation of touch and manual dexterity to be able to palpate muscle tension or to monitor patient pulse.
- Visual acuity to observe patient's gross and fine motor movements to analyze these movements and to read all patient monitors.
- Ability to speak clearly enough to instruct patients and families.
- Ability to hear at a conversational level to understand patient's description of symptoms and response to treatment.
Personal Protective Equipment:
- Follows standard precautions using personal protective equipment as required.
Pay Range
Minimum Annual Salary: $69,600.00
Maximum Annual Salary: $106,132.50

A successful candidate’s actual compensation will be determined after taking factors into consideration such as the candidate’s work history, experience, skill set and education. The pay range displayed does not include any applicable pay practices (e.g., shift differentials, overtime, etc.).
