Cleveland Clinic's Wound, Ostomy and Continence (WOC) Nursing department features nationally certified WOC nurses that specialize in working with patients with select disorders of the integumentary, gastrointestinal, and genitourinary system or who are at risk for pressure injury. Our WOC nurses help patients optimize their quality of life by providing educational resources and managing ongoing clinical support. A physician may refer a patient to a WOC nurse to help patients learn how to care for themselves or until they are comfortable with the type of surgical procedure they had. Our nurses are here to serve as a resource to any concerns and questions from stoma patients.
A caregiver in this position works PRN (as needed) from 7:00AM – 3:30PM.
A caregiver who excels in this role will:

Perform comprehensive nursing assessment for those patients within scope of practice.

Collaborate with the physician and multi-disciplinary care team to coordinate ongoing patient care.

Use data to develop and implement nursing interventions appropriate to meet patient care needs and facilitate the discharge process.

Perform wound/ostomy/continence/skin care based on sound nursing theory, scientific principles, evidence-based practice, professional judgment and individual patient needs.

Follow patient along continuum of care.
-
Assess and triage patients appropriately.
-
Monitor and document patient's condition, effectiveness and timeliness of care and interventions.
-
Evaluate, update and revise plan of care.
-
Delegate appropriate components of the nursing care plan to other members of the nursing team.
-
Assess educational needs of the patient and family/significant other considering cultural issues, age appropriate and other special concerns.
-
Provide teaching and educational material to patient and/or family members/significant others concerning subject matter as appropriate for the specific patient need which may include diagnosis, safe and proper use of equipment, medications, self-care or nutritional needs.

Validate and document patient and family understanding of teaching.

Design, direct, implement and evaluate educational offerings based on knowledge, skills and learning levels of nursing staff for wound/ostomy/continence/skin patient care.

Work with the Nursing Institute to identify learning needs of nursing personnel.
Minimum qualifications for the ideal future caregiver include:

Graduate from an accredited school of professional nursing
-
Bachelor’s of Science in Nursing (BSN)
-
Current licensure as an RN in the state of Ohio
-
Wound Ostomy Continence certifications through the Wound Ostomy Continence Certification Board (WOCNCB) within 24 months of hire
-
Current Basic Life Support (BLS) certification through the American Heart Association (AHA) or American Red Cross
-
One year of recent RN experience
Preferred qualifications for the ideal future caregiver include:

Two years of recent RN experience

Physical Requirements:

Light Work – Exerting up to 20 pounds of force occasionally, and/or up to 10 pounds of force frequently, and/or a negligible amount of force constantly (Constantly: activity or condition exists 2/3 or more of the time) to move objects.
