The Administrator, Home Care provides strategic and operational leadership for Cleveland Clinic Connected Care (CCC), an integrated home and transitional care organization supporting approximately 2,000 patients daily across 19 counties with more than 700 caregivers. CCC delivers a comprehensive range of post-acute and home-based services including home care, hospice, home infusion pharmacy, respiratory therapy, mobile physician services, and facility-based physician practices. This leadership role is responsible for the overall operations of the agency, including revenue cycle management, market network oversight, nursing operations, quality outcomes, retention, budgeting, growth initiatives, and long-term strategic planning. The Administrator oversees 16 direct reports across therapy, pre-certification nursing, and social work functions while collaborating with internal and external stakeholders, including health systems and skilled nursing facilities, to support continuity of care, operational excellence, and patient outcomes across the organization.
A caregiver in this role will work Monday through Friday from 8:00am to 5:00pm and will be on-call for emergencies.
A caregiver who excels in this role will:
- Work closely with the Director of Nursing, directs clinical management, business operations, and all supporting departments.
- Prepare, develop and implement annual operating and capital budgets; monitors ongoing financial performance and makes corrections to plan if necessary.
- Create and support a work environment that fosters quality, accountability, collaboration, and caregiver satisfaction.
- Manage and control financial, operational and quality outcomes of agency by effective use of resources, both human and material.
- Ensure development of all caregivers through effective coaching, counseling and mentoring, leading by example.
- Direct the development and implementation of home care policies and procedures, and contributes to center-wide policies, procedures, and emergency planning.
- Participate in applicable committees. Guides home care leadership meetings and town halls.
- Oversee agency service delivery that is in compliance with regulatory/accreditation standards such as JCAHO, CMS, and ODH, and emulates “best practices”; ensures agency performance that will sustain applicable agency certification/accreditation.
- Develop and maintain a home health compliance plan and OIG work plan in accordance with the Center for Connected Care compliance program.
- Delegate adequate authority to achieve assigned responsibilities and ensures mechanisms for accountability.
- Support and contribute to the development of new programs and services, as relevant.
- Collaborate with the other Center for Connected Care business lines and departments within the health system to deliver clinical services that are in concert with the mission, vision, and values of the organization, yielding positive clinical outcomes and high customer satisfaction.
- Support the development of evidence-based practice through participation in and contribution to the literature, investigational studies, etc.
- Responsible for building and maintaining relationships with referral sources and meeting growth targets of the department.
- Represent Cleveland Clinic Home Care throughout the health system and to outside agencies and organizations.
- Foster a team approach to patient are and encourages collaboration amongst teams.
- Promote and support a continuous improvement environment which incorporates both quantitative and qualitative analysis in order to accomplish effective problem solving and planning; provides outcome reports to key stakeholders at appropriate intervals.
- Must be able to perform all direct care responsibilities as a licensed clinician, as appropriate based on licensure: RN, PT, OT, or SLP
Minimum qualifications for the ideal future caregiver include:
- Bachelor's degree with nursing or therapy concentration
- Master's degree required
- Strong knowledge of the Home Care Medicare Conditions of Participation and JCAHO accreditation standards
- State of Ohio RN or Therapy (Physical Therapist (PT), Occupational Therapist (OT) or Speech Therapy (ST)) license
- Current Basic Life Support (BLS) through American Heart Association (AHA) or American Red Cross
- A minimum of 7 years progressive healthcare management experience
- Minimum of 5 years Home Care experience in a leadership position required
Preferred qualifications for the ideal future caregiver include:
- Nursing or physical therapy experience
- Certification in healthcare/ homecare administration
Physical Requirements:
- Requires sitting, standing and walking for extended periods of time. Requires manual dexterity to perform work on a PC.
- Requires good visual acuity through normal or corrected vision and must be able to hear typical conversation levels through normal or corrected hearing.
Personal Protective Equipment:
- Follows standard precautions using personal protective equipment as required.
Pay Range
Minimum Annual Salary: $112,110.00
Maximum Annual Salary: $170,955.00

A successful candidate’s actual compensation will be determined after taking factors into consideration such as the candidate’s work history, experience, skill set and education.
